Healthy Heart Program

We offer a comprehensive education and exercise program designed for people who have heart disease, particularly those recovering from a cardiac event.  When room available, referrals will be accepted for prevention and require two of the following diagnoses: hypertension, hypercholesterolemia, or diabetes.

The group program is divided into two parts. Phase 1 is an 8-week mini education session followed by a 1 -1.5hr exercise session of cardio including use of the indoor walking track, followed by resistance training exercises with a qualified trainer.  Each participant receives an individualized exercise prescription; therefore we need a recent medical assessment and exercise stress test before the start of Phase I.

Education topics include:

  • Risk factors
  • Diet for a healthy heart
  • Medications
  • Stress management
  • How the heart works
  • Importance of exercise and exercise safety
  • Maintaining motivation

Phase 2 is an 8-week home-based program including weekly contact with the program nurse.

Our program follows guidelines developed by:

  • Canadian Association of Cardiac Rehabilitation
  • American Association of Cardiopulmonary Rehabilitation
  • American College of Sports Medicine

The home-based program offers the same educational framework in the convenience of you home.  The home-based program is divided into 2 8-week phases where participants will have 1:1 weekly telephone appointments to discuss recommended topics and review your weekly exercise.  This program is offered at no cost.

Healthy Heart Registration Information

Participants can now self refer, or have their physician / Internist send referral to enroll.  A participation fee of $40.00 includes print material, education and exercise sessions.
